Youth Clinic Terms & Conditions<\/h2>\n\n\n\n

Code of Conduct<\/strong>: Participants must adhere to camp rules and show respect for coaches, staff, and other players. The camp reserves the right to dismiss any participant for behavior issues without a refund.<\/p>\n\n\n\n

Media Consent<\/strong>: By participating, you grant the camp permission to use photos or videos of your child for promotional purposes.<\/p>\n\n\n\n

Participant Release of Liability, Waiver of Claims, and Authorization of Medical Care<\/strong><\/p>\n\n\n\n

I am the parent or legal guardian of the child (\u201cYouth\u201d) identified during this registration to participate in one or more programs (\u201cPrograms\u201d and each a \u201cProgram\u201d) offered by Sporting JAX Soccer Academy, Inc., a Florida not for profit corporation (\u201cSporting JAX\u201d), and wish for Youth to participate in such Programs. I understand and have explained to Youth that participation in a Program involves dangers or conditions, known or unknown, which are characteristic of, intrinsic to, or an integral part of the activities involved in the Programs and which are not eliminated even if the Sporting JAX acts with due care in a reasonably prudent manner (each, an \u201cInherent Risk\u201d and together \u201cInherent Risks\u201d), which Inherent Risks include, but are not limited to (a) the failure by Sporting JAX to warn Youth or Youth\u2019s natural guardian of an Inherent Risk and (b) the risk that Youth or another participant in the activities involved in the Programs may act in a negligent or intentional manner and contribute to the injury or death of Youth. On behalf of Youth, myself, and any other person who may claim through or on behalf of Youth (collectively, \u201cParticipant Parties\u201d), Youth and I assume all Inherent Risks incidental to, or pertaining in any way to the activities involved in participating in a Program, including, without limitation, Inherent Risks that may result in property damage, mental, physical, emotional, and bodily injuries, and\/or death.<\/p>\n\n\n\n

In consideration for Youth\u2019s participation in one or more Programs, I, on behalf of all Participant Parties and each of our respective heirs, successors and assigns, agree to, and do hereby, discharge, release and waive any claims, damages, lawsuits, and\/or causes of action (together, \u201cClaims\u201d) against Sporting jax, and its respective directors, officers, owners, landlords (and any other owners of the property, land or facilities on which Sporting jax operates or any Program is conducted), affiliates, employees and agents, resulting from an Inherent Risk in the activities involved in the Programs, including Claims for personal injury, death, or property loss or damage. I further covenant not to sue Sporting jax for any Claim released under this Release of Liability, Waiver of Claims and Authorization of Medical Care, and agree to indemnify, defend and hold harmless (including payment of reasonable attorneys\u2019 fees, costs, and insurance deductibles) the Sporting jax with respect to any such Claims.<\/p>\n\n\n\n

In addition to the foregoing, I, on behalf of all Participant Parties, hereby request and a grant permission to Sporting JAX to authorize emergency medical treatment for Youth (including through an athletic trainer, coach, emergency medical technician, nurse, medical treatment facility, and\/or doctor of medicine or dentistry and any personnel associated with the foregoing), if necessary, and I assume personal and financial responsibility for any medical care and treatment that Youth may require as a result of volunteering and participating in the Programs. I also authorize emergency transportation of Youth to a medical treatment facility should an individual listed above consider it to be warranted. The Participant Parties agree to indemnify the Releases from any Claim based on such transportation or other medical care.<\/p>\n\n\n\n

I understand that this Participant Release of Liability, Waiver of Claims, and Authorization of Medical Care is a contract between the Participant Parties and Sporting jax. I sign it of my own free will. I also understand that this contract is severable and if any part of it is held by a court of law to be unenforceable, the rest shall survive.<\/p>\n\n\n\n

NOTICE TO THE MINOR CHILD\u2019S NATURAL GUARDIAN<\/strong><\/p>\n\n\n\n

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF SPORTING JAX USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD\u2019S RIGHT AND YOUR RIGHT TO RECOVER FROM SPORTING JAX IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND SPORTING JAX HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.<\/p>\n\n\n\n
