Excess Liability<\/a><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><a href=\"https:\/\/www.dropbox.com\/scl\/fi\/vgva1l377ubrq3rskgn3i\/participant_injury_or__accident_form_2022.pdf?rlkey=0kn2ghr76rmuk44sx7thvdihu&amp;dl=0\" target=\"_blank\" rel=\"noreferrer noopener\">Participant Injury or&nbsp;Accident<\/a><\/strong><\/p>\n\n\n\n<h1 class=\"wp-block-heading\" id=\"certificate-of-insurance-coi\"><strong>CERTIFICATE OF INSURANCE (COI)<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Local NCYSA Member Associations may request a certificate of insurance (COI) for their locations used for training, scrimmages, games, and tournaments by following the instructions below.&nbsp; This form should be filled out by the Local NCYSA Member Association\u2019s president, executive director, field coordinator, or head registrar&nbsp;<\/strong><strong>ONLY<\/strong><strong>.&nbsp; Forms filled out by any other person will not be approved. &nbsp;&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Expected turnaround time for a new submission is same day (2-6 hours) during normal weekday business hours.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>COIs are only emailed to the one designated person at each Local NCYSA Member Association.&nbsp; &nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>For questions or to request a copy of an EXISTING COI, email\u00a0<a href=\"mailto:keri@ncsoccer.org\" target=\"_blank\" rel=\"noreferrer noopener\"><a href=\"mailto:keri@ncsoccer.org\" target=\"_blank\" rel=\"noreferrer noopener\">keri@ncsoccer.org<\/a><\/a><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>To Request a COI for a New or Missing Location:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Click this link:&nbsp;&nbsp;<a href=\"https:\/\/landing.playershealth.com\/ncsoccer-coi\/\" rel=\"noreferrer noopener\" target=\"_blank\">https:\/\/landing.playershealth.com\/ncsoccer-coi\/<\/a><\/li>\n\n\n\n<li>Enter the Facility\u2019s name and address, etc\u2026.<\/li>\n\n\n\n<li>Choose your Association\u2019s Name in the dropdown menu.<\/li>\n\n\n\n<li>Enter the name of the person filling out the form.<\/li>\n\n\n\n<li><strong>Leave \u201cSpecial Wording\u201d and \u201cEvent Date\u201d BLANK.<\/strong>&nbsp;&nbsp;<em>Only enter text under \u201cSpecial Wording\u201d when it is absolutely necessary.&nbsp;
