The Form is to be completed by <strong>July 1st<\/strong>. <br>This form ensures that all necessary information is up to date. The form must be completed by the club&#8217;s President and submitted once per season.<br><\/p>\n\n\n\n<p>If there are any changes to the club&#8217;s information throughout the season, it is important to update the form and notify Eastern Pennsylvania Youth Soccer promptly.<\/p>\n<\/div><\/div>\n\n\n\n<p><strong>Please Note: Clubs&nbsp;will not be able to start building their league rosters for the season if the Annual Affiliation form is not completed.&nbsp;<\/strong><\/p>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<p>Complete the form in your organization\u2019s GotSport Account.<\/p>\n\n\n\n<p>1. Login at system.gotsport.com<\/p>\n\n\n\n<p>2. Select \u2018Affiliations\u2019 from top menu bar<\/p>\n\n\n\n<p>3. Select \u2018Available Forms (2)\u2019 and&nbsp;the&nbsp;<em>Eastern Pennsylvania Youth Soccer Annual Affiliation Form<\/em>&nbsp;for the appropriate year<\/p>\n\n\n\n<p>4.
